When you arrive to your first consultation there are usually some forms to fill out.

In order to allow you to review the forms and fill them out in the privacy of your own home you may download and print these forms from this page.

After you filled them out just bring them with you to your first visit.

There are at least two ways to download and print these forms:

  1. download the forms to your computer (for example the desktop) and double click them there so that they open in Adobe Acrobat Reader from where you can print them.
    Download in Firefox: Right-click on the icon above and select “Save Link As…”
    Download in Internet Explorer: Right-click on the icon above and select “Save Target As…”
  2. click on each of the links above and the document will open in the Adobe PDF plugin for your browser. (if you don’t have it you may download if free from the Adobe web site) Once the document is open in the browser plugin you can print it to your printer.
